Please read and sign this Program Policies + Health Waiver before participating in any activities at Charlotte Blake Pilates.
 

Program Policies

Open Communication

It is imperative that you keep all communication lines with your instructor open, clear, and upfront. Our instructors will continuously ask you to show them how to best teach you. If they ever say or do anything that does not feel right, please bring it up. We promise to make it right for you and do our best to meet your satisfaction.
 

Confidentiality

Our instructors will honor the confidentiality of everything discussed with you. In the event that an alternate instructor is assigned to you (temporarily or permanently), instructors will discuss all information relevant to your program and progress. Any changes in instructor will be disclosed to you in advance.
 

Package Length

Each session package has a predetermined expiration date based on the date of purchase or the date of your first session, whichever is later. This date may be extended up to one month if requested.
 

Payments

All sessions must be prepaid. Payments may be made online through our online scheduling system or in-person. Other forms of payment accepted include certified checks made payable to Charlotte Blake.
 

Studio Location

In-person sessions will be held at Charlotte Blake Pilates. The Studio is located at 50 West 12th Street, Garden Level (between 5th and 6th Avenue) in New York City. Virtual one-on-one sessions and small group sessions will be held via Zoom Video.
 

Studio Closures + Time-Off

All closures and instructor vacations are accounted for in our online scheduling system and communicated, if known, when scheduling via email. We will do our best to communicate any changes that may affect your scheduled sessions as soon as possible. Booking Sessions

Sessions may be requested through our online scheduling system or by emailing info@cbpnyc.com at least 48 hours in advance. To request session holds for up to two months in advance, please email info@cbpnyc.com. Please note that we do our best to meet your scheduling preferences and appreciate your flexibility when booking.

Confirming Sessions

When booking through our online scheduling system, you will receive confirmation via email when your request has been accepted. When booking or bulk scheduling via email at info@cbpnyc.com, all session holds must be confirmed within 72 hour. Once confirmed, the session(s) will be booked and you will receive official confirmation via email. f we do not receive written confirmation, your sessio(s) will not be booked. This means your session hold(s) will be removed from the schedule and released to other Clients.

Making Changes or Canceling After Booking

All sessions ay be reschedule or cancelled up to 24 hours in advance. Please note that all requests must be received through our online scheduling system or via email at info@cbpnyc.com. Once your change request has been processed, you will receive official confirmation via email. If you do not reschedule or cancel at least 24 hours in advance or if you miss a session without notice, unfortunately you will forfeit that session. When canceling or rescheduling less than 24 hours before your session, please contact your instructor directly via text.

Taking Time Off From Sessions

A 14-day notice is requested before taking vacation or breaks from sessions. All such requests must be received via email at info@cbpnyc.com. Your prefered session block will be held for two consecutive weeks, while you are on vacation or taking a break from sessions. After this grace period, you risk the possibility of your preferred session block being filled by another Client unless the session is paid for in your absence.

Termination

A 14-day notice is requested before ending your program. Please note that all such requests must be received via email at info@cbpnyc.com. In the event of unforeseen circumstances that lead you to terminate your program, we will do our best to ensure that all remaining sessions are used. HOWEVER, WE DO NOT ISSUE REFUNDS UNDER ANY CIRCUMSTANCES.

 

Health + Fitness Waiver

I UNDERSTAND that participating in pilates, training, health and fitness classes, programs, or workshops offered by Charlotte Blake during which I will receive information and instruction about health and fitness. I recognize that fitness programs require physical exertion, which may be strenuous and may cause physical injury and I am fully aware of the risks and hazards involved.
 

I UNDERSTAND that it is my responsibility to consult with a physician prior to and regarding my participation in pilates, training, health and fitness classes, programs, and workshops. I represent and warrant that I am physically fit and I have no medical condition which would prevent my full participation in pilates, training, health and fitness classes, programs, or workshops.
 

I UNDERSTAND that in consideration of being permitted to participate in pilates, training, health and fitness classes, programs, or workshops, I agree to assume full responsibility for any risks, injuries, or damages, known or unknown, which I might incur as a result of participating in the program. In further consideration of being permitted to participate in the pilates, training, health and fitness classes, programs, or workshops, I knowingly, voluntarily, and expressly waive any claim I may have against Charlotte Blake for injury or damages that I may sustain as a result of participating in the program.
 

I UNDERSTAND that I, my heirs, or legal representative forever release, waive, discharge, and covenant not to sue Charlotte Blake for any injury or death caused by negligence or other acts. I have read the above release and waiver liability and fully understand its contents. I voluntarily agree to the terms and conditions above. I further certify that the above information is correct and accurate.
